Thank you so much for being here. I am so excited to provide these full-sized product bundles at a 50% discount for my fellow wellness providers. Each bundle will come with the listed products, a kraft paper bag, a brand-specific matching ribbon, and my business card with a custom discount code for use on my website - everything you need to create gift sets for your patients/ clients.

Here's how it works (PLEASE READ FIRST):

1. Complete this form to submit your provider information. I will be in touch with a discount code for you to use on my website. This is for your use only - please do not share it with your patients/ clients.
   
2. Select your desired bundles from the dropdown menu - you can change quantities on the "My Cart" page before checkout. The minimum order quantity is $200 for an initial order and $100 for subsequent orders.
   
3. Your order will come with my business card and a custom discount code to offer to your clients/ patients to use in my shop. I will use a default code, however, if you would like to have a specific discount code to provide for your clients/ patients, please put that in the Notes at checkout.
